Beyond Inpatient Care2

With the help of our Medical Social Worker, you are encouraged to start planning for discharge care arrangements after you or your loved one has completed all the necessary treatments and rehabilitation at the Hospital. Patient Care Conferences are conducted at least twice during a patient's stay to track each patient's progress and to determine his/her discharge date and care management.

To help ensure you or your loved one's safe discharge home, home visits, home modification assessments and caregiver training are also provived by our Care Team


Patient's and Caregiver's Recovery Journey Guide

Upon discharge, patient will be able to continue receiving the required treatment through the hospital's Day Rehabilitation Centre or its Home Healthcare Services.

Day Rehabilitation Centre
Our Day Rehabilitation Centre (DRC) is a one-stop multi-disciplinary centre situated within Ang Mo Kio - Thye Hua Kwan Hospital. We seek to rehabilitate individuals with disabling conditions due to disease or injuries to regain their function and independence.
Our team of physiotherapists, occupational therapists and speech therapists work in consultation with our clinical team to provide individualised rehabilitation and care plans for our clients.

Home Healthcare Services
Our Home Healthcare services offer
Home Medical, Home Nursing and Home Therapy.

The Personal Data Protection Act (PDPA) protects your personal data while enabling organisations to use your data reasonably to serve you. We, as a community hospital, respect and keep your data safe by:

  • Limiting access to only doctors and healthcare personnel who are involved in your care, and the supporting internal processes.
  • Conducting regular checks to ensure that your personal data is only accessed by authorised persons; and
  • Removing details, as far as possible, that indentifies you when using your data for internal purposes.

Serving you as a Community Hospital

When you seek care at other healthcare and social service providers, we will share relevant data with them through trusted information systems including but not exclusive to the National Electronic Health Record (NEHR) system. We may use your personal data to invite you to participate in suitable care programmes, or shortlist you for participation in relevant research studies.

As a community hospital, we share relevant data and participate in national and multi-agency efforts to:

  • Review healthcare policies and requirements;
  • Review programmes that ensure patient safety and improve the quality of healthcare services;
  • Conduct disease surveillance to address public health concerns; and
  • train future generations of healthcare professionals.

Please be assured that if your personal data is collected, used or disclosed for these purposes, we will protect it as required under the PDPA and other relevant legislation such as the Private Hospitals and Medical Clinics Act.
